Weight-loss injections aren’t for everyone, warns gastroenterologist amid rising fatty liver cases in urban India

New Delhi: India has long been known as the diabetes capital of the world, but another metabolic disorder is rapidly gaining ground – Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD), previously known as non-alcoholic fatty liver disease (NAFLD). Fuelled by sedentary lifestyles, unhealthy eating habits, obesity, stress and rising cases of Type 2 diabetes, the disease is increasingly affecting urban Indians in their 20s, 30s and 40s. What makes it particularly dangerous is that it often progresses silently, with patients unaware that irreversible liver damage may already be underway.

According to Dr. Meghraj Ingle, Senior Consultant – Gastroenterology, Zen Multispeciality Hospital, Chembur, fatty liver disease has become one of the leading causes of chronic liver disease in India and demands far greater public awareness. While modern medicines and weight-loss injections have transformed obesity treatment, they are not a substitute for healthy living and should never be used without medical supervision.

Fatty liver: Silent disease that often goes undetected
Unlike many other liver disorders, fatty liver disease rarely causes symptoms in its early stages. Most people continue their routine lives without experiencing pain, jaundice, swelling or fatigue. However, beneath the surface, excess fat gradually accumulates inside liver cells, triggering chronic inflammation.

Over time, this inflammation can lead to fibrosis (scarring), cirrhosis and, in severe cases, liver failure or liver cancer if left untreated. Experts say the absence of symptoms is one of the biggest reasons why the disease often remains undiagnosed until significant damage has already occurred.

Medical literature also shows that MASLD has become one of the most common causes of chronic liver disease worldwide, with India witnessing a particularly sharp rise due to increasing obesity and diabetes rates.

Why urban Indians are at greater risk
Doctors attribute the surge in fatty liver disease to modern urban lifestyles. Long working hours, prolonged sitting, minimal physical activity, frequent consumption of processed and high-calorie foods, irregular sleep schedules, stress and alcohol consumption all contribute significantly to liver fat accumulation.

The risk becomes substantially higher among people living with:

  • Type 2 diabetes
  • Obesity or excess abdominal fat
  • High cholesterol
  • Hypertension
  • Metabolic syndrome

Fatty liver isn’t just alcohol-related disease
One of the biggest myths surrounding fatty liver disease is that it only affects people who consume alcohol.

In reality, a large proportion of patients diagnosed with MASLD do not drink alcohol at all. Indian experts say South Asians are genetically predisposed to storing fat around internal organs—including the liver—even when they appear relatively lean.

This explains why many Indians with a normal Body Mass Index (BMI) may still develop fatty liver, a condition often referred to as “lean fatty liver.”

Can fatty liver be reversed
The answer, experts say, is yes -particularly in the early stages.

Weight reduction remains the single most effective treatment. Even moderate, sustained weight loss has been shown to reduce liver fat, improve inflammation and lower the risk of progression to fibrosis.

Doctors recommend focusing on:

  • Gradual and sustained weight loss
  • A balanced, calorie-controlled diet
  • Regular physical activity
  • Adequate sleep
  • Stress management through yoga or meditation
  • Better control of diabetes, blood pressure and cholesterol

Lifestyle modification continues to be the cornerstone of treatment and remains the first-line recommendation in international liver disease guidelines.

Weight-loss injections: Effective, but not for everyone
The growing popularity of injectable weight-loss medications such as Semaglutide and Tirzepatide (Mounjaro) has sparked tremendous interest among people looking to lose weight quickly.

These medications have demonstrated significant benefits for obesity, Type 2 diabetes and, in carefully selected patients, improvements in fatty liver disease. Emerging research and international guidelines increasingly support their use in patients with obesity and metabolic disease under specialist supervision.

However, Dr Ingle cautions that these medicines are not cosmetic weight-loss solutions.

Many people now seek these injections simply to lose a few kilograms without understanding the medical criteria or potential risks. Before prescribing them, doctors evaluate several factors including body weight, metabolic profile, existing illnesses and overall health.

The medications can also produce side effects such as:

  • Nausea
  • Vomiting
  • Gallbladder stone formation
  • Pancreatitis (inflammation of the pancreas)
  • Rare thyroid-related complications
  • Prevention is still best medicine

Doctors say India’s rising burden of obesity, diabetes and metabolic disorders means fatty liver disease is likely to become an even bigger public health challenge in the coming years.

The good news is that unlike many chronic diseases, fatty liver is often preventable—and in many cases reversible—if identified early and managed appropriately.

Regular health check-ups, screening for high-risk individuals, maintaining a healthy weight, exercising consistently and controlling diabetes and cholesterol remain the most powerful tools to protect liver health.

As awareness grows around obesity medications and new treatment options, specialists stress that there is still no shortcut to a healthy liver. Sustainable lifestyle changes and not quick fixes continue to offer the strongest defence against one of urban India’s fastest-growing silent diseases.
